  • Benefit premium deductions are paid a month ahead and can only be added, changed, or canceled during open enrollment or due to a qualifying event due to marriage, divorce, loss of coverage, or birth/ adoption of a child. Please see Page 3 of the health insurance form for detailed information.

If you are at a doctor's office or pharmacy and are declined services, call our service center, press option "5" for urgent and a representative will assist you.

If you have questions about member or dependent eligibility or enrollment in one of the state retiree insurance plans, or COBRA, please call our Service Center at  800-253-9981 or email us at benefits.administration@tn.gov. We can help you over the phone or by email. If you need an in-person counseling meeting, please ask us to make an appointment for you. Appointments must be scheduled at least 48 hours in advance.

  • Premier Preferred Provider Organization, or PPOHigher premiums, but lower out-of-pocket costs for your deductible, copays and coinsurance.
  • Standard PPOLower premiums than the Premier PPO, but you’ll pay more out-of-pocket for your deductible, copays and coinsurance.
  • Limited PPOLower premiums than the other PPOs, but you’ll pay more out-of-pocket for your deductible, copays and coinsurance compared to the other PPOs.
  • Local Consumer-driven Health Plan with a Health Savings Account, or CDHP/HSALowest premiums, but you pay your deductible first before the plan pays anything for most services, and then you pay coinsurance, not copays. Go to the CDHP/HSA page to learn more.

ID Cards: Newly enrolled members or members who make changes to their current health plan option will receive new ID cards. Members can request additional ID cards by contacting their carrier or by using the carrier’s mobile app.

The SBC may be found at www.tn.gov/ ParTNersForHealth/summary-of-benefits-and-coverage no later than Sept. 1. The digital newsletter contains much of the same information. To get a SBC paper copy, free of charge, call 855.809.0071. Please include your name, complete mailing address, and name of the SBCs you want: State and Higher Education Plan, Local Education Plan, or Local Government Plan.
  • The Plans are required by law to maintain the privacy of protected health information and to provide you with notice of our legal duties and privacy practices with respect to PHI.   • Find the Notice Regarding Wellness Program at tn.gov/ParTNersForHealth under Wellness, or email benefits.info@tn.gov to request a mailed copy of the Wellness Program Notice.
  • Benefits Administration does not support any practice that excludes participation in programs or denies the benefits of such programs on the basis of race, color, national origin, sex, age, or disability in its health programs and activities.
